~~ NASA Forms 1699 and 1699A
When a request is made for reasonable accommodation, it must be documented on NASA Form 1699, "Request for Reasonable Accommodation".
When a decision is made, it must be documented on NASA Form 1699a, "Disposition of Reasonable Accommodation Request".
Both forms must be forwarded to the Equal Oppportunity Programs Office, Code 120, in a timely manner. These forms can also be found in Appendix A of NPR 3713.1, "Procedures for Providing Reasonable Accommodation for Individuals with Disabilities" which can be found, above.
The two forms are available below, in three formats.
The first is the text of the forms in Word, the second is the Form itself in PDF, taken directly from the NPR, and the third is in writeable, or fillable, PDF, that you can use online.

Requests for assistive technology can be made directly to the Computer/Electronic Accommodations Program (CAP)~ at the Department of Defense. Please contact the EOPO Disability Program Manager for further guidance in using this vehicle.
